How We Achieve and Sustain Transition Solutions

To support state and local programs to achieve results in transition, we will:

• Create a vision and plan for change
  We facilitate interagency leadership teams, use data to identify needs and guide planning, build consensus on priority goals, and develop and manage strategic action plans.
• Move strategic plans into action
  We develop effective communication and marketing strategies to achieve system-wide buy-in, align services through resource mapping, and identify and use research-based practices to support program improvements.
• Achieve visible short-term wins
  We use data to guide daily program management, make mid-course corrections, and communicate early accomplishments.
• Support state and local planning and scaling-up efforts
  We facilitate local interagency team development, resource mapping, and professional development on research-based practices, program design, and effective use of data to guide local decision making and planning.
• Evaluate and sustain results
  We consult on state performance measures, and train on program evaluation methods and effective use of data to communicate results.
